Athletic Clearance Registration

Every student-athlete is required to be cleared through Home Campus. The athletic clearance process includes all the necessary forms required by the NFHS, CIF, state of CA, Diocese, and school. Your coach may require additional forms at their discretion.
We highly recommend getting your physical exam completed by your primary care physician. If that is not available to you at this time, we have a few other options for you. We have partnered with Marque Medical to ease the process of obtaining a physical exam.
**IMPORTANT**: To ensure full compliance with diocesan directives and CIF guidelines, all student-athletes are required to complete a new physical dated no earlier than May 19. This will ensures medical clearance remains valid for the entire 2026-2027 school year. Physicals must be submitted by June 13 for athletes to be eligible to participate in any summer activities.
Students will not be permitted to participate until:
- A physical that covers the 2026-2027 school year has been submitted, and
- All required online forms have been completed on Home Campus
The Rosary Athletics Department can only accept physical exams completed by an MD, DO, NP, or PA under the supervision of an MD or DO. Physical exam forms must also be stamped to be considered valid and have full participation box checked. If a physical exam does not meet these requirements, it will be returned to the athlete. Please do not turn in your athletics physical exam to the main office.
