Athletic Clearance Registration
Every student-athlete is required to be cleared through Home Campus. The athletic clearance process includes all the necessary forms required by the NFHS, CIF, state of CA, Diocese, and school. Your coach may require additional forms at their discretion.
We highly recommend getting your physical exam completed by your primary care physician. If that is not available to you at this time, we have a few other options for you. We have partnered with Marque Medical and The Drop-In Doc (Dr. Adam Schulte) to ease the process of obtaining a physical exam.
*IMPORTANT* : Your online Home Campus registration and physical exam must be turned in prior to any sports participation. Please verify that the physical exam is dated on or after May 31, 2023. This will ensure that your athlete is covered for the entire school year. To submit your athlete’s physical exam, upload a copy on Home Campus when completing the online athletic clearance process.
The Rosary Athletics Department can only accept physical exams completed by an MD, DO, NP, or PA under the supervision of an MD or DO. Physical exam forms must also be stamped to be considered valid. If a physical exam does not meet these requirements it will be returned to the athlete. Please do not turn in your athletics physical exam to the main office.
